The authorized representative shall submit, with its application, the following documents:

System-generated/downloaded Cover Sheet for Amendment;
System-generated/downloaded, signed and notarized or apostilled/authenticated Amendment Form;
Monitoring Clearance from the Compliance and Monitoring Division (CMD) or from other Department/s of the Commission;
Certification and/or Favorable Endorsement of an appropriate government agency or from other Department/s of the Commission, if necessary.

Note: The documents listed above are basic requirements. Additional documents may be required depending on the type of provisions, which will be indicated during the application process.
